Instructions for Downloading Online Forms

We try to make things as convenient as possible for our families, volunteers and potential Board Members. That's why we have made all the important forms you will need accessible via the internet and in two versions. For the fastest processing of your applications or forms, please consider downloading a Microsoft Word version, saving your changes on your computer, and e-mailing us back the document as an attachment.

If you are unable to do this, feel free to click our PDF version of the file, print it out, and fax, mail or bring it into our office. For a copy of Adobe Acrobat to read PDF files click here.

If you do choose our Word copy of the documents, we have provided a helpful download guide below.

First click on the document name. Example: "Child Application"

Click Save and a "Save As" box pops up.

We suggest selecting your Desktop as the location and changing the document name to "Child's Name_Application" for every document. Example: "Sally_Application" "Sally_ParentApplication" etc.

When the file completes downloading, this box pops up:

Select Open and Microsoft Word will Open the document

Because all of our files are locked and can only be changed by our Administrators, this pops up:

Select Read Only and your file will open completely:

Every box that has a greyish rectangle in it is available to make changes. Type in all your information, save the document, and e-mail us at bbbsfc@verizon.net with the document attached to the e-mail. It would also be helpful to include your and your child's name in the body of the e-mail message.
